Foster/Adoption Forms

Everyone is welcome to attend our clinics.  You may have the opportunity to meet the foster parents there.  They can best answer your questions and help you determine if a particular cat or dog would be a suitable member for your family.  We are also very happy to work with you in between clinics.  But please keep in mind that we have no central location where our cats and dogs are held.  You will have the best opportunity of meeting most, if not all, of our cats or dogs at one time and in one place except at an adoption clinic.

The application and approval processes for new foster homes and potential adopters are identical.  Once your application is received, we will process it as quickly as possible.  We need your help to accomplish this.  We are unable to process incomplete or inaccurate applications, so please take a few minutes to give us all requested information, especially contact information for you, your veterinarian, and if applicable, your landlord.  Prior to adopting, we will require proof of identification such as a driver’s license or passport, and proof of residency.

H.O.P.E. requires that all of your cats and/or dogs are or have been spayed/neutered.  Your pets must have a history of regular veterinary care. Home Visits are required at the discretion of the foster home or Board of Directors.  We may or may not bring the specific pet you are interested in adopting to your home during this visit, depending on the availability of the foster family.  We may ask you to have your local shelter or rescue contact us about conducting your home visit for us.  And H.O.P.E. is always happy to return the favor!

Final decisions regarding placement may be at the discretion of the Board of Directors.  Please be aware that the welfare of our cats and dogs is our top priority, and all placement decisions are made based on what we feel is in the best interests of our pets.  If your application is not accepted, it does not mean that we are saying that you would not be a good cat or dog owner, but simply that we believe the pet would be a better match in a different home.  If we believe that we are not likely to have the type of pet you seek within a reasonable period of time, we will gladly refer you to other organizations.

To continue, see the application forms listed below.  You may print an application out and mail it to us, or use our online form.  We do not have a fax. Our online forms have a “send” button.  All you need do is complete the form and send.  We look forward to working with you to help you find the pet of your dreams!

Your e-mails and telephone calls are important to us.  Please keep in mind that we are all volunteers.  We ask that you be patient, and we will get back to you as soon as possible.  Thank you for contacting us!
